The post holder will work as part of team from our Mitcham office providing a client focused service committed to delivering a high-quality care using a multidisciplinary approach. The post holder will enable clients who have been discharged from hospital or who are requiring assistance to remain living at home to regain their independence, manage their long-term conditions and support clients during periods of personal crisis.
The post holder will be expected to work without direct supervision for all or most of the shift

HOURS: 30- 40hr/week
LOCATION: MITCHAM
Key Responsibilities
Clinical/Professional Duties:
- To carry out delegated work as set out in the client’s care plan initiated by the co-coordinator.
- Support with clients’ rehabilitation on returning home from a stay in hospital.
- Proactive and responsive support to prevent clients’ admission or re-admission into hospital.
- To only undertake tasks the post holder feels competent to perform as directed by the Care Manager.
- To work unsupervised, using own initiative as required.
- To be adaptable and confident to deal with unexpected situations.
- To prioritise workload.
- To act in a professional and confident way at all times.
Client/Client Care:
- To support clients with all daily living tasks which promote independence and wellbeing.
- To support clients with food preparation and the monitoring of food and fluid intake as directed.
- Adhere to, review and report the effectiveness of the client’s recovery plan and undertaking delegated tasks such as catheter care, stoma care, monitoring of vital signs such as Blood pressure and simple dressings (once trained and assessed as competent for each client).
- To assist clients with personal care duties which promote dignity and respect at all times.
- Assistance with medication as directed.
- To be able to work autonomously in people’s homes, carrying out specified care plan duties.
- To enable clients to manage their activities of daily living in accordance with their individual care plans.
- Participate in appropriate health enhancing activities in order to meet the needs of clients.
Communication skills:
- To maintain accurate client records.
- To deal sensitively with a range of clients in different situations (eg. Distressed/tearful)
- To communicate clearly and concisely both verbally and in written documentation.
- To be able to identify and inform coordinator of changes in client’s condition.
- To work as part of a team.
- To ensure that all documents are completed accurately and correctly using IT equipment provided.
- Report immediately and appropriately any significant changes in circumstances of the clients
- Use the provided IT systems effectively and efficiently.
- Report immediately any adverse incidents or complaints to the line manager.
Education and Research:
- To maintain and develop skills, knowledge and competencies and attend statutory training sessions and demonstrate willingness to train to develop clinical skills.

- To obtain the Care Certificate within 6 months of taking on the role if not already got it.
Education and Research:
- Input data and information accurately and completely using the latest versions of policies and procedures in manual and electronic systems.
- Ensure the necessary precautions are adhered to when transferring information, only disclosing client identifiable information in line with the Caldicott and GDPR.
